Purpose:

This application form must be submitted by all registered entities in ADGM if they want to appoint and/or cease the Data Controller Contact Person, which means either the Data Protection Contact Person and/or Data Protection Officer as applicable.

Data Protection Contact Person:Provide the following information

· Appointment Date/Cessation Date

Data Protection Officer:

Same information as above plus the additional supporting documents listed below

Pay Attention:

1) The appointment of a data protection contact person is mandatory for all ADGM registered entities.

2) Appointment and cessation may be submitted in the same application form.

3) The appointment of a data protection officer is required in the following circumstances:

Timeline:

As soon as practicable after the change occurred, prior to filing the annual data protection renewal form, in case there are any changes to the data held on file by the ADGM RA.

Timeframe:

KOUNTED 1-2 working days including drafting the online application, provided all the relevant information has been provided

· Passport Copy & UAE residence visa copy of the Data Protection Officer as applicable

· Educational Certificates of the Data Protection Officer (foreign language documents must be accompanied by a certified true copy to the English language)
